Dear Mr. Nelson,
 
I just read your story.  Whoa! I am so sorry for your pain, your long journey in search of a definitive diagnosis.....and of course the outcome. I am impressed by your drive and your desire to help others like "us". Thank you!
 
My name is Sandi. I am a 39 yr. old (40 in a few weeks) widow and Mother of 3 who is suffering from ATypical Facial Pain w/ Trigeminal Neuralgia Shocks.  My second severe bout with the disease began a few weeks ago. I began feeling a vague toothache in the lower right quadrant of my mouth and I just knew what was coming next. My teeth are great.......I've worked for a group of Dentists for 18 years. Within days of my first little symptom, "it" struck while I was brushing my teeth lasting for several seconds. I dropped to my knees, cradling my face and cried my heart out. I just couldn't believe this was happening again!
 
I felt so scared! I went to work and made it through the day with just a few attacks. That was a Wed.. That night it hit me hard while I slept. @ 2:22am I awoke feeling like a bolt of lightning had severed my lower jaw.
 
I panicked. The pain would not stop. Finally after 1 hour and 23 minutes my pain subsided, only to wake me again @ 5:10am. I stayed home from work that Thur. afraid to move my head. The blasts of pain came frequently and with great force. I turned on my computer and began looking for info. on TN. I read and read attempting to keep my mind busy and decided to cut out all the middle men and make an appt. with a Neurosurgeon. I phoned the handful of PPO providers I was allowed to choose from and the soonest appointment available was April 27.
 
I begged and pleaded with each receptionist, but to no avail. Friday was worse and by Saturday I was ready to shoot myself! My Sister and Mother came over to help me and my children and by late Sat. afternoon my pain was completely out of control. It nearly knocked me down. My Sis & Daughter loaded me into the car and rushed me to the ER where I was forced to sit and wait for nearly an hour. When I was finally taken back, not one single nurse had even heard of TN and I swear they looked at me writhing in pain, as I attempted to speak like I was out of my mind.

A physicians' assistant came into the little room and ordered a shot of Torodol. I almost laughed. Then the Dr. arrived. At least she knew about TN! As she was talking with me I exploded, my arms went straight up and my body convulsed. She ordered Morphine. That's like sticking a band-aid on my face, but they shot me with it 3 times. It didn't even take the edge off. SO then they applied a Fentanol patch to my back, gave me a 300mg dose of Neurontin, a script for that and Ambien to help me sleep and sent me home because "they had done all they were able". I admit I was pretty "goofy" and with the combination of drugs, I eventually passed out only to wake in the wee hours of the morning vomiting.  By early evening of the next day I was taken back to the ER. I was severely dehydrated and was advised that my TN had caused Labrynthitis (Vertigo). I was given an IV to rehydrate and medication for dizziness and nausea. What a weekend!
 
My attacks of pain continued, fiercely pounding my face until I wished I would just die. I forced liquids, but even sipping water would more often than not send me into another attack. My poor children just looked at me with pain and confusion in their eyes. I felt guilty and so sorry for them. They were afraid to touch me, afraid they might cause me pain. I lay for days in the darkness of my bedroom just praying for God, for someone to stop the pain!
 
Then finally it was time to see my new Neurologist. I had an attack in the waiting room and another as the nurse called my name. Tears streamed down my cheeks as I waited.
I went to Dr. C feeling hopeful, in desperate need of compassion. What I received however, was a lot of paper work, a quick exam, an explanation of "Yes, you have TN, there are surgical procedures available, you're most likely a candidate, take these pills, see a Neurosurgeon"..... and I was out the door. My daughter and I felt  disappointed.  My new medication is Trileptal, 300mg. for 6 days then 600 mg./day. The Dr. advised I combine this with a Narcotic (Percocet) and Ativan to help me sleep. {I now rattle when I walk}, but I am sleeping better.

 I have fairly good days and then I have horrible days. On my good days I only have 4-5 attacks, but I'm always tired and I feel so "drugged" all the time. In the past 2 weeks I've worked a total of 3 days, I've prepared 1 complete meal, and my eldest Daughter has become my 11 year old Son's Mom! I hate this disease! Hate it hate it hate it!!!!!! I've lost 12 pounds in 10 days. It's a real good thing I have some extra weight to lose. Perhaps by the time this is over with I'll really have my "girlish figure" back!  Ooh La La!!! Just joking! I'd rather have a big behind any day!
 
Today is April 22, 2005. This all began around March 21, 2005. Because of interruptions due to pain, children, the telephone, and just feeling bad in general, it has taken me days to type my story. I first wrote to Brian on April 17.
Okay............I saw the Neurologist today. I like him. He was compassionate, and seemed well educated on the subject of TN. We discussed MVD

surgery and both agreed it is the best choice of treatment for me. He ordered another MRI

with contrast and small cuts which I will have on Monday, April, 25. My surgery is scheduled on May 12, 2005 (as long as everything looks good on the MRI). My daughters say they feel relieved, my son gave me a "double high-5", and as for myself..............I feel some relief, like I can almost see a light at the end of the tunnel. I feel a bit nervous as well, not only about surgery on my head, my brain, but about things such as how we'll get by, my job, will I feel well enough to get everything and everybody organized before my surgery. My list of worries could go on and on. This disease not only causes PAIN...........it creates complete chaos. I've already used all of my sick days and 4 of my 12 vacation days. Dr. says my recovery time is 3-4 weeks. I definitely can't afford NOT to have surgery, so now I have to figure out a way to pay the bills and keep my household running because I'm having surgery.

With the cost of medication, and co-pays for all my Dr. visits I've already blown my already tight monthly budget. So, we cut back on groceries, no fun outings for a while, and I certainly hope my son's tennis shoes hold out (I've warned him very firmly this time...."DO NOT stop your bicycle with your toes"!). The utility bills will be paid late of course, but we'll get by somehow.....we always do. One thing we have an abundance of in my home is L O V E. No it doesn't pay the rent, but it warms the heart. The love and closeness I share with my children is a cherished commodity. We have something quite wonderful, something I don't see in far too many other homes. Some may have fat pocket books and go on fancy vacations, but I'd rather have my beautiful, healthy, amazing children and the L O V E, compassion, respect, and adoration we feel for one another......... Without all this......my pain would surely kill me.

 
I am not asking for sympathy. In fact, I don't want sympathy.......compassion Yes of course. I am only telling "my" story and how TN affects me. We are all unique and come from different walks of life. I am a hard working single Mom with a decent job who struggles now and then. I have an older car and worn furniture, but my home is clean and welcoming. My kids are happy & healthy and I smile A LOT (when I'm not in pain that is). I've had a tough life, but I'm tougher and I've gone through some things that could have, should have knocked me down. I refuse to surrender to this disease. I admit I have my "days" when I suffer with excruciating pain when I "think" I can't go on, but I have too much to live for and so do you!
 
I realize my story is much like many others' stories. I am not different. I may suffer more than you or less than you, but we are all in "this" disease together. I have come to the realization that I have a unique responsibility to help others who suffer as I have.....as I am today. I believe reaching out to others is imperative to my own healing. Mr. Brian Nelson is a perfect example of someone who "reaches out". Reading his story, his medical reports, and knowing without asking all the hours he has put into research while he suffers gives true meaning to the words "giving", "unselfish", and all those other great words that I can't think of because my mind is so fuzzy. (meds.)
 
My computer is very ill and will no longer allow me access to the internet,
but I am so thankful I was able to come across Mr. Nelson's story. I also got the opportunity to meet 2 women and 1 young man (fellow sufferers) via the internet after posting a very short version of my story on one of the TN web sights. Now I have 4 "pain pals". Keeping in touch through e-mail has helped me tremendously. We have to stick together and support one another. Mr. Nelson has offered to post my story on one of his pages. {Thank you Brian}

Shirley's Stories                  No. 2

I was having TN ( Trigeminal Neuralgia pains before I realized I had it. For about a year I would get lightning like pains every now and then, including while out in public.
 

But over time, I would say about a year - the pains came closer together and I finally realized that this was something serious. I had read about TN in our home copy of THE MERCK MANUAL, which is a reference book that MDs use and it also can be accessed on the Internet. The TN pains got very bad over the President's Day weekend and of course the clinic wasn't open then or on the Monday following.

I took the maximum dosage of extra strength acetaminophen each day and when I did get to the clinic, my primary MD scolded me, saying that this could be very damaging to my liver.  If I had it to over again I would go to the ER at the hospital and they probably could have given me something more effective. Over the counter pain killers do not cover the pain of TN.

I did diagnose myself - my attorney husband having had a client years ago  who had TN - and I recognized the pain as being what used to be called "tic douloureaux". This self diagnosis was confirmed by my primary MD and the neurologist to whom I was referred. The neurologist and I wanted to have the Balloon Compression surgery done right away, but the neuro surgeon advised a "drug trial" to which I agreed. Had I to do it over I would have had the surgery immediately. The Balloon Compression is an outpatient surgery under general anesthetic. I felt no pain at any time. The two drugs I was taking were tapered off over the next month. I went in to the hospital early in the morning and was home by noon. My daughter took me there and home again.

 I was advised to rest for 24 hours but I resumed my usual home activities that evening and it didn't cause me any trouble. I was given pain medication at the hospital to take home, but didn't find it necessary to take more than a couple of the pills.There was only a band aid at the right hand corner of my mouth with just a speck of blood on it!

I do have "dysesthesia", which means "bad feeling" - for me this consists of a sort of constant spasm of the right hand side of my face, but never any "tic" of the eye. I never had that at any time. About 5% - 7% of those having the Balloon Compression have this permanent after effect and one is fully informed of the possibilities before surgery.I will take this rather than the excruciating TN pains - and those of you who have TN will know what I mean.

If you are having lightning like pains that come at unexpected times or are triggered by such things as brushing your teeth, flossing, eating, speaking, touching your face, being in a light breeze either cold or warm, or any other action that uses your face, your might have TN and should see your doctor and get a referral to a neurologist. Don't waste any time!

 Shirley's HUSBAND'S STORY.  posted 4-21-05
 My husband does not have TN, but he is dealing with the results of the polio that he had at age 17. He is now 82 and had a very bad pneumonia more than a year ago. His breathing muscles, diaphragm, and intercostals, were compromised by the polio years ago and the pneumonia did further damage to these breathing muscles.

 We didn't know for a while whether he was going to survive, but he did - with excellent medical care and many prayers. Whole congregations were praying for him! He has had a tracheotomy and will always have a breathing tube in his neck and must be on oxygen all the time. He now can be off the ventilator for about 12 hours during the day and then back on for 12 hours at night.
 
We are grateful for his being off the vent for 12 hours daily, since no one thought he would be able to do this, but the respiratory therapist where he is now thought he would be able to be "weaned" off and undertook to do this. This was successful and we are so glad that this is so and thanks to Ron, the caring respiratory therapist.
 
 We feel that it is family emotional support and constant presence that has kept my husband going.
It is hard to be living away from home, but with family visiting often and bringing books, magazines, and newspapers,  playing cribbage and Scrabble with him, and taking him outside in the wheelchair for walks, he is getting as close to normal living as we can manage.
 
 If you have a family member in a health care facility, be sure to visit often and spend as much time with that person as possible. And while you are there, get to know the other residents - many of them never have any visitors.

 MY FAVORITE HOBBY AND WHY.  posted 4-21-05

Reading has always been my joy - and continues to be my favorite pastime. The public library has always been my favorite place! Nothing makes me happier than bringing home a big armload of delicious books. When I was growing up, I read all the books in the children's library in Appleton, Wisconsin, and so the librarian issued me a card to the adult library at about the age of nine or ten.

Now that's an avid reader! I even walked all the way there and back, and that was about a mile. We didn't have a bus in those days. This was about 1939 - I am now 75.

One of the things that bothers me most in a big city is the amount of trash lying around. If we all picked up one piece of trash per day, think of how much better things would look.

We need to say hi or hello to people we pass on the street. Many are surprised to get a greeting!
